Migraines, fatigue, brain fog, heart palpitations, insomnia and sometimes even a fever are among the symptoms that disrupt her day-to-day life.\u00a0<\/p>\n\n\n\n<p>Lisette Duarte still needs an inhaler eight months after her COVID-19 infection, and damage to her voice box occasionally leaves her struggling to speak. She can\u2019t return to work, so she relies on Medi-Cal for her health coverage.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/long-term-effects\/index.html\">Long COVID<\/a> is a mysterious, debilitating and difficult-to-diagnose compilation of post-infection problems that have affected perhaps millions of Californians. And health experts fear that the toll will be especially harsh for Latinos, African Americans and low-income residents, who already <a href=\"https:\/\/calmatters.org\/health\/coronavirus\/2022\/02\/covid-black-california-deaths\/\">face a disproportionate impact from COVID-19 <\/a>as well as less access to quality health care.<\/p>\n\n\n\n<p><a href=\"https:\/\/www.survivorcorps.com\/pccc-ca\">At least 20 specialized, post-COVID programs<\/a> have been set up at medical centers in California to help treat these long-haul patients. But they already are overburdened, and experts fear that long COVID could go largely unaddressed or misdiagnosed in people who have few resources.&nbsp;<\/p>\n\n\n\n<p>\u201cWhat we know is that because the greatest amount of exposure happens in low-income communities, they will have the highest burden of long COVID,\u201d said <a href=\"https:\/\/medicine.ucsf.edu\/people\/neeta-thakur\">Dr. Neeta Thakur<\/a>, a&nbsp;pulmonary and critical care doctor at UC San Francisco.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote has-background\" style=\"background-color:#ffda83\"><blockquote><p>People with long COVID &#8220;feel frustrated. They\u2019re told it&#8217;s anxiety or to be patient, that symptoms will get better on their own.\u201d\u201d<\/p><cite>Dr. Jeffrey Hsu, UCLA Health\u2019s long COVID program<\/cite><\/blockquote><\/figure>\n\n\n\n<p>Long COVID patients often need a team of specialists because cardiac, respiratory and neurological problems have been documented. As a result, people with Medi-Cal or no health insurance may struggle to find proper care and pay for expensive tests.<\/p>\n\n\n\n<p>Some vast regions of California have few doctors experienced in diagnosing and treating long COVID conditions. Of the 20 post-COVID clinics and programs in California, according to a <a href=\"https:\/\/www.survivorcorps.com\/pccc-ca\">crowdsourced list collected by a patient advocacy group,<\/a> nearly all are in the Bay Area, Los Angeles, Orange County and San Diego. Of those listed, only one is in the Central Valley. None are in the San Joaquin Valley, an eight-county <a href=\"https:\/\/www.chcf.org\/blog\/hospitals-central-valley-flooded-covid-19-patients\/\">region where hospitals were inundated<\/a> during previous COVID-19 infection waves.&nbsp;<\/p>\n\n\n\n<p>The general experience (of patients) is that they\u2019ve been seen by numerous providers in the community. They feel frustrated,\u201d said <a href=\"https:\/\/www.uclahealth.org\/providers\/jeffrey-hsu\">Dr. Jeffrey Hsu<\/a>, a cardiologist who is on a team of physicians treating patients at <a href=\"https:\/\/connect.uclahealth.org\/2022\/02\/14\/long-covid-treatment-program-symptoms\/\">UCLA Health\u2019s long COVID program<\/a>. \u201cThey\u2019re told it&#8217;s anxiety or to be patient, that symptoms will get better on their own.\u201d<\/p>\n\n\n\n<p>Studies have estimated that more than <a href=\"https:\/\/health.ucdavis.edu\/news\/headlines\/studies-show-long-haul-covid-19-afflicts-1-in-4-covid-19-patients-regardless-of-severity\/2021\/03\">one in four COVID-19 patients<\/a> experience symptoms that last for months, known as \u201clong haulers.\u201d In California, that could mean as many as 2 million people among the 8.5 million COVID-19 cases may have long COVID, including more than 100,000 African Americans and one million Latinos.<\/p>\n\n\n\n<p>But it\u2019s hard to know how many people actually suffer from long COVID, largely because it is still being defined: The U.S. Centers for Disease Control and Prevention says symptoms are considered long-term if they last more than four weeks after an infection; the World Health Organization says it\u2019s 12 weeks.<\/p>\n\n\n\n<p>The California Department of Public Health is collaborating with UCSF and UCLA on <a href=\"https:\/\/www.covidinspire.org\/\">a national study<\/a> tracking the aftermath of COVID infections through patient surveys. Without providing details, the department also said it is working on partnerships with the University of California system to expand access to post-COVID information and care.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-demand-for-long-covid-care-exceeds-capacity\">Demand for long COVID care exceeds capacity<\/h3>\n\n\n\n<p>At UCLA, the long COVID team includes four primary care doctors who evaluate patients and then connect them to the appropriate specialist \u2014 cardiologists, neurologists, pulmonologists, psychiatrists and others.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-embed alignright is-type-rich is-provider-infogram wp-block-embed-infogram\"><div class=\"wp-block-embed__wrapper\">\n<div class=\"infogram-embed\" data-id=\"4f05b8d0-2ea7-41da-b90e-345712acbfa2\" data-type=\"interactive\" data-title=\"Long COVID recovery programs\"><\/div><script>!function(e,n,i,s){var d=\"InfogramEmbeds\";var o=e.getElementsByTagName(n)[0];if(window[d]&&window[d].initialized)window[d].process&&window[d].process();else if(!e.getElementById(i)){var r=e.createElement(n);r.async=1,r.id=i,r.src=s,o.parentNode.insertBefore(r,o)}}(document,\"script\",\"infogram-async\",\"https:\/\/e.infogram.com\/js\/dist\/embed-loader-min.js\");<\/script>\n<\/div><\/figure>\n\n\n\n<p>Hsu said demand for the program is high. UCLA\u2019s program has received more than 300 referrals, but only has capacity to see 200 patients.<\/p>\n\n\n\n<p>About half of the patients in UCLA\u2019s program are already UCLA Health members, while the other half are referred from other providers around Los Angeles, he said. He worries about patients who don\u2019t have insurance since a lot of expensive tests are involved, and those battling symptoms who live in areas without access to providers who have been trained to identify long COVID.<\/p>\n\n\n\n<p>\u201cWe just have to imagine that the patients we see are the ones who have the resources to get to us,\u201d Hsu said.&nbsp;<\/p>\n\n\n\n<p>Patients with long COVID could go to their primary care doctors, who can then consult with experts. But another challenge is the long-standing uneven distribution of primary and specialty care physicians across the state. For instance, the ratio of active physicians per 100,000 people in the Inland Empire and San Joaquin Valley is about half of the ratio in the Bay Area, according <a href=\"https:\/\/www.chcf.org\/wp-content\/uploads\/2018\/06\/CAPhysicianSupply2018.pdf\">to a 2018 report<\/a> from the California Health Care Foundation.&nbsp;<\/p>\n\n\n\n<p>Duarte, who lives in East Los Angeles, had to push hard for months to get the right help for her ongoing conditions.\u201cNobody was listening to me,\u201d she said. It took her switching doctors and filing complaints, but last month she got a referral to a long COVID program.<\/p>\n\n\n\n<p>\u201cI definitely feel like I\u2019ve received substandard care,\u201d Duarte said. She wonders if it\u2019s because she\u2019s covered by Medi-Cal, California\u2019s insurance for low-income residents.<\/p>\n\n\n\n<p>Research has shown that in many cases, the health symptoms of people of color&nbsp; and low-income people are not taken seriously, leading to misdiagnosis and prolonged poor health.&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote has-background\" style=\"background-color:#ffda83\"><blockquote><p>\u201cI\u2019ve changed providers several times. I am being treated for several conditions that are likely going to be with me long-term, if not forever.\u201d<\/p><cite>Angela Meriquez Vazquez, long covid patient in Los Angeles<\/cite><\/blockquote><\/figure>\n\n\n\n<p>For example, African Americans receive worse care than whites in 43% of the quality-of-care measures, such as effective treatment, <a href=\"https:\/\/www.ahrq.gov\/sites\/default\/files\/wysiwyg\/research\/findings\/nhqrdr\/2021qdr.pdf\">set by the federal government<\/a>. Latinos receive worse care in 36% of the measures compared to whites.&nbsp;<\/p>\n\n\n\n<p>That inequity could be particularly problematic for patients with long COVID, since it\u2019s difficult to diagnose and treat properly. Similar to the disparities seen in other chronic conditions like diabetes and hypertension, providers say they anticipate unequal treatment of the conditions that spring from long COVID.&nbsp;<\/p>\n\n\n\n<p>\u201cJust as the state tries to put an equity framework around vaccine distribution, having some kind of equity framework around limited resources when it comes to treatment may be helpful,\u201d said Kristen Azar, a registered nurse and scientific medical director for the Sutter Health Institute for Advancing Health Equity.&nbsp;<\/p>\n\n\n\n<p>Vazquez works for the Children\u2019s Partnership, an advocacy organization for child health, so she understands how to navigate health systems and advocate for herself. Photo by Lauren Justice for CalMatters<\/figcaption><\/figure><\/div>\n\n\n\n<p>Among the conditions she is being treated for: <a href=\"https:\/\/www.webmd.com\/heart-disease\/atrial-fibrillation\/postural-orthostatic-tachycardia\">postural orthostatic tachycardia syn<\/a><a href=\"http:\/\/www.dysautonomiainternational.org\/page.php?ID=30\">drome<\/a>, a circulatory disorder that leaves her lightheaded. She doesn\u2019t leave her house much because even the slightest social activity leaves her exhausted.<\/p>\n\n\n\n<p>\u201cWhat would have been a normal day, let\u2019s say working and then going to a brewery for happy hour \u2014 if I did that today, I would absolutely go to bed with a huge fever and the shakes,\u201d she said.&nbsp;<\/p>\n\n\n\n<p>Last year, Vazquez became president of <a href=\"https:\/\/www.wearebodypolitic.com\/\">Body Politic<\/a>, an advocacy group that serves as a resource and support group for long COVID patients. She advocates for training and education about long COVID among Medi-Cal providers, so that low-income patients can receive proper care.&nbsp;<\/p>\n\n\n\n<p>\u201cThe state needs to make sure that all Medi-Cal providers and beneficiaries have information about long COVID, &#8221; Vazquez said. \u201cWhen someone tests positive, at the next follow up appointment, doctors should be asking about long-term symptoms.\u201d&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote has-background\" style=\"background-color:#ffda83\"><blockquote><p>\u201cThe COVID-19 pandemic has the potential to be an enormous disabling event, in addition to being a cause of acute illness and death.&#8221;<\/p><cite>Dr. Erica Pan, state epidemiologist<\/cite><\/blockquote><\/figure>\n\n\n\n<p><a href=\"https:\/\/calmatters.org\/health\/coronavirus\/2021\/07\/medi-cal-covid-vaccinations\/\">Medi-Cal patients have a lower vaccination rate<\/a> than California\u2019s general population, leaving them more vulnerable to severe disease and long-term symptoms. About 56% of Medi-Cal enrollees had received at least one dose of the vaccine, compared to 83% of the general population, according to the state\u2019s February <a href=\"https:\/\/www.dhcs.ca.gov\/Documents\/COVID-19\/DHCS-COVID-19-Vaccine-Stats.pdf\">vaccine update<\/a>.&nbsp;<\/p>\n\n\n\n<p>Blacks and Latinos in California also <a href=\"https:\/\/www.kff.org\/coronavirus-covid-19\/issue-brief\/latest-data-on-covid-19-vaccinations-by-race-ethnicity\/\">have a lower vaccination rate than whites<\/a>.<\/p>\n\n\n\n<p>People who are vaccinated are less likely to report some symptoms of long COVID, such as muscle pain and fatigue, according to a <a href=\"https:\/\/www.nature.com\/articles\/d41586-022-00177-5\">small study in Israel<\/a>. But vaccinated people and those with mild infections can still wind up being COVID long haulers.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-with-debilitating-symptoms-many-can-t-return-to-work\">With debilitating symptoms, many can\u2019t return to work<\/h3>\n\n\n\n<p>Vazquez gets to work from home, a privilege not everyone with long COVID has. \u201cThat reduces my exposure,\u201d Vazquez said. \u201cBut essential workers, there\u2019s no way that in my current health I could be on my feet and check groceries everyday.\u201d<\/p>\n\n\n\n<p>About 2.37 million people may be out of work at some point due to long COVID, according to Dr. Gregg Vanichkachorn, medical director of the Mayo Clinic COVID Activity Rehabilitation Program, who spoke at a state legislative hearing earlier this month.&nbsp;<\/p>\n\n\n\n<p>While the state doesn\u2019t have its own estimate, \u201cit\u2019s likely that a large number of Californians will be affected,\u201d Dr. Erica Pan, the state\u2019s epidemiologist, said at the hearing.<\/p>\n\n\n\n<p>\u201cThe COVID-19 pandemic has the potential to be an enormous disabling event, in addition to being a cause of acute illness and death,\u201d she said.&nbsp;<\/p>\n\n\n\n<p>Duarte is one of the patients who said she can\u2019t return to work. She is a consultant who helps education and autism nonprofits develop programming. She said her voice box issues, which she thinks could be from excessive coughing, her brain fog and occasional reliance on oxygen tanks make it impossible for her to work.<\/p>\n\n\n\n<p>\u201cI wish we had access to case managers who attend appointments with you and help you navigate this,\u201d she said.
